Registration Process 

Welcome to Swansea Public Schools! 

During the school year, families can register their children by calling and making an appointment with one of the School Counselors.  Please contact Mrs. Grauer for last names A-L or Mrs. Mathieu for M-Z. 

During the summer, please contact Heather Montigny or Gail Donmoyer in the main office at 508-675-0116.

Please fill out packet before your appointment. 

**Only complete the medication administration forms if your child will require medication during the school day.** 

Please read carefully.

*Please keep in mind that if the residency requirements are not met your child will not be registered. Please read the residency statement thoroughly before entering. If you are unsure about a piece of documentation, please call the school for further guidance and we will be happy to help. 

*In addition to the paper work we ask you to fill out, please provide a copy of your child's most recent immunizations, recent physical and if your child has an IEP or 504 plan with the previous district, a recent copy. Children may be registered but may not be scheduled until we have a recent copy of these documents.
