Hello and welcome to Summit Public Schools!
In order to meet the requirements set forth in N.J.A.C. 8:57-4 and District Policy 5320 for continued enrollment in the Summit School System, new students must submit the following:
2) Proof of vaccine/immunization status
3) A physical exam (Physical exams must be performed by a U.S. doctor within 365 days of your student’s first day of school)
Forms accepted are Universal Child Health Record or Summit Public Schools Medical Evaluation Form
Please note, a tuberculosis test is required if transferring from a country with a high incidents of TB.
All medical documentation should be sent to the school nurse prior to the first day of school to prevent exclusion.
