Below is a link to a video from Athletic Director Kyle Sasala regarding fall sports. All parents and student-athletes participating in a fall sport are required to watch and sign off on this video through Final Forms before student-athletes can participate in tryouts and practices. 

All student-athletes must be “all green” on final forms, meaning all student forms and parent forms are signed and a valid physical is on file with the athletic office. No exceptions. If your student-athlete does not have all three items completed by Friday, August 1 they will be held out of all team activities. Below is the walk through on how to create a Final Forms account. If you need assistance setting up a Final Forms account, registering your student-athlete for a sport, need a blank physical form, etc. please contact the athletic department.
