“Creating Focused Interventions for Patients with Heart Failure in the SNF"
The purpose of this project is to created focused interventions in the form of a clinical pathway to address the gaps in OT practice for the HF population. In the SNF, therapeutic exercise and ADLs overshadow interventions concerning cognitive impairment, psychosocial wellbeing, and lifestyle management. The project aims to incorporate holistic and evidence based interventions which are imperative for the success of HF clients after discharge.
The first goal of our project is to identify areas of treatment that OTs should address to best serve HF clients within the SNF setting. Some examples of areas that might be identified based on our research and needs assessment include: effective knowledge translation for client and caregiver education, cultivating self-efficacy and social support, managing/monitoring symptoms and/or vitals, and others. A secondary goal of our project is to create a clinical pathway to guide current OTs working within SNFs on how to address these areas of need for clients with chronic HF through standardized means. Along with evidence based interventions, the clinical pathway will include resources such as suggested assessments and a summary of evidence for therapists.
The first step we did was creating a concept mapping on the topic. Based on the concept map, we reviewed the literatures to find evidence that will support our project.
After the concept mapping, we wrote 'literature review' and build our thesis based on the evidence that we found through the article search. Through the literature review, we found gaps and needs in SNF for HF clients and started to create the clinical pathway.
In Spring 2019, our capstone team applied grant and received seed money from seed money from the California Occupational Therapy Foundation and from the ENspire Seed Money Grant Program from Ensign Services. “Creating Focused Interventions for Patients with Heart Failure in the SNF".
Presentation on the Clinical Pathway
Our capstone team presented the clinical pathway at the Ensign Affiliated SNF in Sonoma County. The presentation included lecture and workshop. During the lecture part, we presented gaps and needs in the SNF and evidence that we found through literature review. During the workshop, we presented the handouts and activities that we created for the HF clients and introduced how to implement the activities during the OT interventions. The audience participated in the workshop activities and gain knowledge how to use them through hands-on experience.
Clinical Pathway for HF Clients in SNF
These are the final version of our clinical pathway for HF clients in SNF. The clinical pathway has four different phases.
The phases can be weeks or days depending on the client’s stay, conditions, and on their needs. The phases can be used flexibly and each phases can be weeks or days depending on the client’s stay, conditions, and on their needs. Majority of the interventions can be done in concurrent, group, or individual. OT can make judgement whether the session going to be a concurrent, group, or individual. Client with severe cognitive impairment will benefit more from individual session accompanied with caregivers. If the session is designed to be a group or concurrent, the clients will have time to build social relationships with other clients and share informations.
Over the summer, our capstone team was invited to present our graduate thesis project to Ensign Therapy DOR Meeting what was held in Newport, CA. We presented the clinical pathway through 1 hour PowerPoint presentation and poster presentation.
