Abstract form

Informed consent

This is a crucial form if you are conducting the research yourself on human participants

Gives the participants a background of your project, the risks, and asks for their signatures and consent of you conducting research on them. You need this or else you will be disqualified if you did research yourself on humans.

Sample IRB, which is like an informed consent form, but if we did research on humans at a regulated research institution or used data that came from a regulated research institution

This form is a sample, so often the research institution will provide you with their own IRB.

Qualified scientist

Quick form your mentor will fill out to explain their background in science and give information directly from them on what was involved in your study

Regulated research institution

Completed by the supervising adult at your research facility if you did research at one. It details what the conditions were like in the lab, what equipment was used and how it was used by the student, and how independent the student waas in their work.

You discuss rationale, hypothesis, procedures, risks, questions, and many other aspects. This is so important because you always need plan before you begin a project
