ATHLETIC CLEARANCE FORMS & INSTRUCTIONS

The following information must be completed in order to be cleared to play a sport at Franklin High School:

1.  Register on Family ID.

2. Fill out all Physical Forms linked below and submit to Family ID.

3. Once cleared (24-48 hours), you are allowed to attend tryouts and practices for your sport. If you have any questions or issues uploading documents, please contact John Bava via email @ jbava@stocktonusd.net.
