Immunizations

We will let you know if your child is missing any immunizations needed for school entry and will help in any way we can to make sure your student is properly immunized. Immunizations keep children safe from disease and are required by the State of Vermont.  School nurses have to report immunization compliance status to the Vermont Department of Health.  

Immunization exemption related forms and information

• Religious Exemption Form to be completed every year. Please contact the health office for more information. 

• Medical Exemption form to be signed once by your child's Primary Care Provider:  Vermont Dept. of Health Medical Immunization Exemption Form
