REQUIRED FORMS FOR ATTENDING

Required for each participant. You will provide information about the participant, family contacts, insurance provider and health information.

BOX CITY COVENANT

Youth events provide us with lots of great opportunities to learn, share, and grow. We have the chance to deepen our faith and grow our relationships with God and each other. In order to get the most out of each event, we make some promises to ourselves and to each other about who we will be and how we will act.

PARENT/GUARDIAN PERMISSION

The forms gives youth participants permission from a parent/guardian to participate in Box City 2024.

SPONSOR FORM

In addition to being an educational and life-changing experience, Box City 2024 is a fundraiser. Each participant must have 2 sponsors in order to participate in Box City 2024. See the GIVE page for more details about our goal. All funds raised will go to support the construction of a warming center in Pottstown managed by Pottstown Beacon of Hope.
