Substitute Form Developer submissions are divided into two teams at the Colorado Department of Revenue.
Paper forms (sometime referred to as 1D forms) team.
e-File forms (sometimes referred to as 2D forms) team.
Vendor's replicating paper forms only, may email the e-File team to request a waiver.
Vendor's replicating e-file forms only, must submit a version to the paper forms team. The paper forms team will review the layout of the form and additional content that will not be reviewed by the e-File team.
The requirements for each team varies. Please make sure to read the Specifications for the type of form that will be replicated each year for additional details.
Letter of Intent (DR 5029) is a form that is completed by the substitute form developer vendors community to signal their intent to replicate Colorado tax paper and/or e-File forms. Both form review teams use this form to update and add substitute form developer vendors information to their systems. This form must be completed every year a substitute form developer intends to replicate any of Colorado's tax forms. Download the entire zip folder for 2024 Letter of Intent (DR 5029) here.
Submission Guidelines:
To avoid returns or rejections, ensure all submissions meet the following criteria:
Check the current year forms pages and determine:
Is the form status is marked as "Final"?
Does the revision date on the CDOR form match the revision date on the SFD form?
Prepare the output PDF version of the form:
Contains all pages of the form.
Pages are in numerical order (page 1, 2, 3...)
All pages have variable dummy data.
Contains two (2) samples. (Scanline forms may require three (3) samples).
Variable data matches the expected formatting.
(Name, CAN, SSN, Dates, Amount of Payment, ETC.)
SFD who align variable text vertical centered may use "XXX" or "999" in specific area's. Please see the specifications document for more details.
Create an email:
Title of the email should be the software form developers NACTP number, the type of submission (new, resubmission, or courtesy copy), and the date of the submission.
The body of the email should list the forms being submitted at this time.
Attach each output PDF to the email.
Email the submission to DOR_FD_Vendor@state.co.us.
Submission time frames:
The first submission of forms marked as "Final", is due within six weeks of original posting.
Some forms will have line reference updates later in the season. A courtesy copy is due within two weeks of the updated posting.
Scanlines:
Specifications for scanlines, a listing of the forms that require scanlines, and samples of the check-digit calculation are provided in the specifications document. The following forms contain scanlines:
DR 0104BEP, DR 0104EP, DR 0105EP, DR 0106EP, DR 0112EP, DR 0158-C, DR 0158-F, DR 0158-I, DR 0158-N, DR 0900, DR 0900C, DR 0900F, and DR 0900P.

Forms should be complete and in the order presented in the 2D Test Package.
Version date must match the current 2D Test Package and Layout.
