CMS identified phishing scams for medical records. This may include scammers faxing you fraudulent medical records requests to get you to send patient records in response; see example (PDF).
When you review any requests, look for signs of a scam, including:
Directing you to send records to an unfamiliar fax number or address
Referencing Medicare.gov or @Medicare (.gov)
Indicating they need records to “update insurance accordingly”
A scam request may include:
Poor grammar, misspellings, or strange wording
Incorrect phone numbers
Skewed or outdated logos
Graphics that are cut and pasted
If you think you got a fraudulent or questionable request, work with your Medical Review Contractor to confirm if it’s real. Submit medical documentation through the Electronic Submission of Medical Documentation (esMD) system or CMS medical review contractor secure internet portals, when available.
Enhancements have been applied to the three Outcome and Assessment Information Set (OASIS) Final Validation Reports available in the Internet Quality Improvement and Evaluation System (iQIES). The three enhanced reports are listed below:
iQIES system-generated OASIS Agency Final Validation Report
OASIS Agency Final Validation Report – user-requested report
OASIS Submitter Final Validation Report – user-requested report
Changes:
The Submitter User ID field now contains the user's Health Care Quality Information Systems (HCQIS) Access Roles and Profile (HARP) ID, instead of an iQIES system-specific numeric ID.
Improved processing time of the IQIES system-generated final validation report. Users will notice that the reports are available in a much shorter period of time following completion of the processing of the OASIS records.
The file naming convention of the system-generated final validation report has been updated as follows:
OASIS Agency Final Validation - Submission ID XXXXXXXXX CCN NNNNNN - YYYYMMDD CCCCCC
where CCCCCC is a unique numeric IQIES system ID.
The system-generated final validation report is now distributed to all users associated to the provider’s CCN, not just to the submitter of the assessment records.
The sort order for the report has been updated to include sorting by Error Severity (Fatal errors display first, then Warning messages), and then by Item ID of the items triggering the specific error/warning.
