Five-Star Ratings of Nursing Homes – Provider Rating Report for July 2023
The July 2023 Five Star Nursing Home Provider Preview Reports (report 173) have been distributed into each facility’s shared folder in the CASPER Reporting application in QIES where users have been accustomed to accessing the Five Star reports in the past. These shared folders are labeled in the following manner:
[State Code] LTC [Facility ID].
Where:
State Code = the two-character State Code of your facility
LTC = Long Term care
Facility ID = State-assigned nursing home facility ID used for submitting MDS 3.0 records
Upon accessing the above folder, select the file name that includes the “5star_report173_20230701” reference.
CMS had originally planned to distribute these reports in iQIES starting in July; however, the distribution of the Five Star Nursing Home Provider Preview Reports in iQIES will begin in August 2023 instead.

The iQIES S&C Known Issues log is a list of identified system functionality problems or issues that describes the issue, area affected, and the issue’s status. The log is updated on an ongoing basis.
You can access this log on the secure login area of the QIES Technical Support Office (QTSO) website, where S&C system issues are only viewable by State Survey Agencies and CMS staff.
To view the contents in QTSO:
Log into HARP
Click the “Request a Role in HARP”
Select QTSO for the program and organization
Select QTSO CMS/State User Role

SNF/NH Provider Preview Reports for October 2023 Care Compare Refresh for July 2023
Beginning in July 2023 forward, all Skilled Nursing Facility (SNF) Quality Reporting Program (QRP), Nursing Home (NH) Five Star and the MDS Provider and Resident Preview Reports will be available in the Internet Quality Improvement and Evaluation System (iQIES). The July SNF QRP and Nursing Home Five Star Provider Preview Reports coincide with the October 2023 Care Compare refresh for NH and SNF QPR data. No longer will providers access the Certification and Survey Provider Enhanced Reporting (CASPER) Reporting application in the Quality Improvement and Evaluation System (QIES) for these reports.
The MDS Provider and MDS Resident Preview Reports were distributed in iQIES on July 12, 2023, and the SNF QRP Provider Preview Reports will be available on Friday July 14, 2023. Providers will also receive the monthly and quarterly Nursing Home Five Star Preview reports in IQIES, when available.
To locate your new reports in iQIES, please follow the instructions listed below:
Log into iQIES using your Health Care Quality Information Systems (HCQIS) Access Roles and Profile (HARP) user ID and password.
If you do not have a HARP account, you may register for a HARP ID.
In the Reports menu, select My Reports.
From the My Reports page, locate the MDS 3.0 Provider Preview Reports folder. Select the MDS 3.0 Provider Preview Reports link to open the folder.
Here you will see the list of reports available for download, including those identified above.
Once located, select the report name link to view the report data.
