With the help of our contractors, we revised the SNF ABN, Form CMS-10055, and the form instructions. The SNF ABN form and instructions have been revised and are located in the download section. The form is available for immediate use but on 10/31/2024, the form will be mandatory.
As an SNF you must issue a notice to Original Medicare (Fee for Service - FFS) beneficiaries to transfer potential financial liability before the SNF provides:
an item or service that is usually paid for by Medicare, but may not be paid for in this particular instance because it is not medically reasonable and necessary, or
custodial care.
For Part A items and services: SNFs use the SNF ABN as the liability notice.
For Part B items and services: SNFs use the Advance Beneficiary Notice of Non-coverage (ABN), Form CMS-R-131. The ABN and information on this notice can be found at by selecting the link.

September 10, 2024 - The American Society of Colon and Rectal Surgeons (ASCRS) has issued updated guidelines, published in The American Society of Colon and Rectal Surgeons Clinical Practice Guidelines for the Evaluation and Management of Chronic Constipation.
The document stresses the need for personalized care, a multidisciplinary approach citing both strong and conditional recommendations. "Given the range of specialties that manage constipation, a collaborative approach is often warranted to achieve optimal patient outcomes," the guidelines committee emphasizes.
The MDS 3.0 Manual Individual Assessment Correction/Deletion Request form may be used for changes to the A0410 such as an A0410=2 to an A0410=3 or A0410=3 to A0410=1 for individual assessments the same way it is sent in for an individual deletion.
If you want to change all assessments from A0410=2 to A0410=3, the MDS 3.0 Facility Level Item A0410 Change Request form will be sent. The MDS 3.0 Facility Level Item A0410 Change Request would change 'ALL' assessments to A0410=3, not just individual assessments as the MDS 3.0 Manual Individual Assessment Correction/Deletion Request would do. The changes would be made by iQIES. The form is submitted if you wish to remove the assessment from the system as well. Let me know if you have any questions.
