Absence Request Form
Leave Request (FMLA, Maternity, etc)
Service Record Request
If you have planned absences due to doctors appointments, events, etc. you must submit the request at least 2 weeks prior or ASAP.
If it is an emergency absence (ill, family emergency, etc) you still have to submit the absence request at your earliest convenience. If you are absent before or after a holiday/PD day you must submit a doctor's note. You must also email and text all admins of your absence.
MAKE SURE TO ENTER YOUR REQUEST ON ADP PLATFORM AS WELL.
This is important so that substitutes can be scheduled effectively.Â
The Leave Request Form is used when you have to take a long period of time off due to medical, maternity, bereavement, etc. Once this form is filled out you must submit it to Mrs. Castaneda and I will email it to HR. HR then will contact you to provide you the steps that follow.
