What are Transition Services
Transition services means a coordinated set of activities for a child with a disability that—(1) Is designed to be within a results-oriented process, that is focused on improving the academic and functional achievement of the child with a disability to facilitate the child’s movement from school to post-school activities, including postsecondary education, vocational education, integrated employment (including supported employment), continuing and adult education, adult services, independent living, or community participation
What are Transition Services based on
The student's PINS (Preferences, Interests, Needs, and Strengths)
How does the school know my child's PINS?
The school will conduct annual AATA (Age Appropriate Transition Assessment) on the student to identify a student's PINS. The goals and activities in the transition plan need to be aligned to this information. Parent and student input is essential in this process. Transition services need to be student centered and based on each student's individual PINS.
