EMPLOYEE'S HEALTH DECLARATION
The Inter-Agency Task Force for the Management of Emerging Infectious Diseases (IATF-MEID), with the approval of Malacanang, lifted on 16 May 2020 athe enhanced community quarantine (ECQ) in Iloilo City and classified the city as low risk area. On the other hand, the Iloilo City LGU has announced that despite the lifting of the ECQ, will continue to impose limited restrictions, mindful that the Covid-19 threat is not over.
While the University is one with the National and Local Governments in the fight against Covid-19, and there being a need to resume operations and having no prohibition against it, the University adopted the original Safety Guidelines on 18 May 2020. On 15 August 2020, the Department of Trade and Industry and Department of Labor and Employment issued Joint Memorandum Circular No. 20-04-A, Series of 2020, or the “DTI and DOLE Supplemental Guidelines on Workplace Prevention and Control Of Covid-19”. Hereby, the Work Safety Guidelines were amended.
Per the Work Safety Guidelinbes, accomplishing this form is mandatory to all SPUI personnel. Any misrepresentation shall be considered as a grave offense under the SPUI 2018 Comprehensive Guidelines on Compensation, Personnel Benefits and Discipline and may be punished with “Withholding of privileges, benefits, or other emoluments, forfeiture of salary, suspension, or any combination thereof from twenty-one (21) days to thirty (30) days; and/or demotion of rank, or termination from employment for the succeeding offenses”.
