The Transition Medicine Clinic at Baylor College of Medicine provides primary medical care and social support services to the growing population of adolescents/young adults (beginning at the age of 19) with a chronic childhood illness or disability (specifically neurodevelopmental disorders) as they graduate from the pediatric to the adult healthcare system. The only clinic of its kind in Texas, and one of only a few in the nation, The Transition Medicine Clinic assists these patients and their caregivers with accessing adult medical care, navigating the adult healthcare system, and maintaining or identifying alternative social services that are so critical for optimal health. We also serve as a unique resource to other healthcare professionals and the surrounding community, and as advocates for this patient population regarding local, state and federal healthcare initiatives.
Anyone interested in establishing with our clinic must go through the intake process, and our medical director must provide approval before we can assist with scheduling an appointment.
