Apprenticeship Credits

PROCESS:

Prior to beginning

1. Review AP-601 Appendix B

2. Download Apprenticeship Credit Policy - 2023

   • Student completes Sections 1 - 3 from Appendix C: Apprenticeship Credit Proposal Template

   • Supervising teacher, student and employer complete Section 4 from Appendix C: Apprenticeship Credit Proposal Template

   • Ensure all names & signatures are on page 14

   • Scan/email Appendix C: Apprenticeship Credit Proposal Template (pages 10 - 14) to bruce.mellesmoen@spiritsd.ca 

3. You will receive confirmation that the project has been approved or denied

Upon successful completion of 100 hours (minimum)

1. Refer to Section 8 from page 14 of Appendix C: Apprenticeship Credit Proposal Template

2. Fill in all required information

3. Scan and send to bruce.mellesmoen@spiritsd.ca and cc penny.walter@spiritsd.ca  

   • You will need the signed copy returned prior to step 4

4. Download Form 8.1 (below)

5. Fill in all required information

6. Complete FORM 6A

7. Scan and send Appendix C: Apprenticeship Credit Proposal Template AND Form 8.1 and Form 6A to dataservices@spiritsd.ca 

   • Please cc: bruce.mellesmoen@spiritsd.ca and penny.walter@spiritsd.ca  

Retain all documentation for Apprenticeship Credits for a minimum of five (5) years
