Please print all materials on the application checklist below and submit in a FOLDER

• On the front of the folder include: Full Name, Grade, and Student ID Number

• For LAST NAMES A - L: Submit your application to Miss Harris in room A301

• For LAST NAMES M - Z: Submit your application to Mrs. Wahl in room A302 

• DUE DATE:  SUBMIT NO LATER THAN Thursday, September 18, 2025 by the end of the school day

Application Materials

1. NHS Application Checklist

• Make sure this is signed by you and your parents

2. School Involvement Form- Activity and Athletics Tracker

• Make sure it is clear which activities you participate in at SAHS

• These must be school-sponsored activities and athletics

3. Community Service Tracker

• See the Community Service Guidelines  for more information on qualifying community service

• Clearly describe what YOU did during this volunteer time

• You must list the name of the organization, contact person, and contact information

• Make sure the activity was done as community service and not as a requirement of another school or non-school organization

4.  Community Service Question- 

• Please answer one question listed on the document 

5. Coach/Advisor Referrals- at least 2 referrals from a coach or club advisor

**Coaches and advisors will submit these directly to the NHS advisor and will not be included in the application packet you submit.**

• Please remind your advisors to return these to Mrs. Wahl or Miss Harris by  September 18, 2025 

• Please submit at least 2 referrals from a coach or a club advisor  

• Check the status of your referrals with your coach/advisor as well as the NHS advisor
