Please print and submit all materials on the application checklist in a FOLDER
Full name, grade and student ID number on the front of folder
Give your application to Mrs. Wahl in room A302 or Miss Harris in room A301 by Thursday, September 19, 2024 by the end of the school day.
Make sure this is signed by you and your parents
2. School Involvement Form- Activity and Athletics Tracker
Make sure it is clear which activities you participate in at SAHS
These must be school-sponsored activities
Make sure your community service meets expectations and guidelines.
Clearly describe what YOU did during this volunteer time.
You must list the name of the organization, contact person, and contact information.
Make sure the activity was done as community service and not as a requirement of another school or non-school organization.
4. Community Service Question-
Please answer one question listed on the document.
5. Coach/Advisor Referrals- at least 2 referrals from a coach or club advisor
Coaches and advisors will submit these directly to the NHS advisor and will not be included in the application packet you submit.
Please remind your advisors to return these to Mrs. Wahl or Miss Harris by September 19th, 2024.
Please submit at least 2 referrals from a coach or a club advisor.
Check the status of your referrals with your coach/advisor as well as the NHS advisor.
