- Proof of Payment: * Deposit Slip/Transaction Receipt

- Photo* (half body - white attire with red background)

- Good Moral Certificate

- Financial Clearance

- Baptismal Certificate

- Marriage Contract of Parents (if available)

- ESC or QVR Certificate for JHS (Gr.8 - Gr.10) & SHS Learner from private school
