FORMS and due dates BELOW WILL BE EXPLAINED AT ORIENTATION
**Please make sure you have your Palomar ID number saved/memorized as you will be asked to add that number on each form (NOT your SMHS ID number)**
Enrollment Application (this is different from the K12 form you already filled out)- due Friday, January 23rd
Statement of Cooperation - due Friday, January 30th
Monthly Report (Time Sheet) - there is a column on the sheet to enter hours for each month
Please do not turn in without totaling up all of your hours for the month.
Please email the final timesheet as a PDF (NOT as a png or jpeg file) with signatures from you AND a supervisor. You can screenshot your Jan, Feb, and March & April hours but the final sheet must be sent as a PDF.
January hours due February 6th (start adding hours as of January 26)
February hours due March 6th
March hours due April 3rd
April hours, May hours AND FINAL TIMESHEET due April 24th (fill out & estimate your April hours through April 30th and fill out and estimate your May hours through May 22nd)

Grades will be submitted to Palomar College and sent to San Marcos High School to be posted to your transcript for Spring '26
