Please complete the Geneva Housing Questionnaire and the Swiss Check Form by following the instructions below. 

Note: the Housing Questionnaire, the Swiss Check Form for Health Equivalency and the passport photo must be submitted by the September 9th deadline. 

Housing Questionnaire

There are three different housing options available to students in Geneva. Please read the description of these options and then complete the Housing Questionnaire by no later than September 9th.

Swiss Check Form for Health Insurance Equivalency

Please complete the Swiss Check Form for Health Insurance Equivalency by following the link below. Note: some information has been entered for you.

INSTRUCTIONS

• 1. Download the form and TYPE the following information in Part 1: last name, first name, phone number, email address, date of birth, nationality, your sex, and marital status.

  2. Leave type of permit blank, but list that it will be valid from January 23, 2026, and that your stay in Switzerland will end in June 21, 2026.

  3. Leave Part 2 blank.

  4. Print both pages of this form double-sided, and sign and date the box labeled "signature of policyholder" in the middle of the page at the bottom of Part 2. DO NOT sign at the bottom of the page where it says "Stamp/seal and signature of insurer."

  5. Submit completed form  via email to the Lewis Global Studies Center (studyabroad@smith.edu).

One .jpg Photo

• • Please use the US Travel.State.Gov photo tool to send a passport-style photo to the Lewis Global Studies Center (studyabroad@smith.edu) as a .jpg via email with a file name as follows: LAST NAME_first name. The .jpg photo just needs to be of your face against a white background.
