MAT Info - Licensure File Schedule

The evaluative materials below must be delivered in person to the office of the Associate Director of Teacher Education based on the schedule below. Interns cannot graduate and become licensed without a complete and accurate certification file.

All forms MUST be signed by all appropriate parties. Incomplete material sets (see below) will not be accepted.

These materials are due in your licensure file the week of October:

Observation 1 - College Supervisor (conference with Supervisor, Mentor, and Intern)

Professionalism Checklist #1 – Mentor Teacher (completed then discussed with Intern)

These materials are due in your certification file the final week of January:

Observation 2 - College Supervisor (conference with Supervisor, Mentor, and Intern)

Observation 3 - College Supervisor (conference with Supervisor, Mentor, and Intern)

Midpoint Evaluation - Including SPA criteria

Professionalism Checklist #2 – Mentor Teacher (completed then discussed with Intern)

These materials are due in your certification file the first week of April:

Observation 4 - College Supervisor (conference with Supervisor, Mentor, and Intern)

Observation 5/6 - College Supervisor (conference with Supervisor, Mentor, and Intern)

Professionalism Checklist #3 – Mentor Teacher (completed then discussed with Intern)

FINAL Evaluation - including SPA criteria
