Instructions to upload to your CRA My Account
(To be uploaded to the primary caregivers account. This would be the person receiving the monthly Canada Child Benefit for the child. If two people are receiving the CCB, separate letters should be generated and uploaded for each person on their respective accounts.)
Select "Submit documents" on your main CRA page to get to the Submit documents area.
Once in the Submit documents area, select "Submit documents" again which will bring you to a new page.
Click "Start"
Do you have a case or reference number? Select, "No"
Under Topic select, "Canada Child Benefit (CCB), GST/HST Credit and related programs"
Under Indicate the reason for your submission, select, "Send documents or forms to support my Canada child benefit (CCB) and related programs"
Under Specify the reason for your submission, select "Additional Canada Child Benefits related requests "
Click the "Attachments" button and then the "Browse" button to navigate to the PDF you downloaded and select the file
Put in a description and click "Attach file(s)"
If the child was approved for the DTC when they were 18 or older, ensure you also attach a copy of the Notice of Determination that will be found in their CRA mail area.
Click "Next" and then Click "Submit"
Your document submission will now appear in your submitted documents area.
