Scout Medical Forms

Both scouts and leaders must submit BSA Health Forms every year.

Forms A & B

  • Required for all participants.

  • Required for all activities - including Troop meetings!

  • Can be completed by a parent or guardian.

Form C

  • Required if you are attending a long-term camp or Any event lasting 72 hours or more (e.g. long weekend).

  • Must be completed by a doctor (includes Urgent Care facilities).

Instructions:

    1. Download the BSA Health Forms here:

      1. BSA Annual Health Forms

    2. Make a copy for your records.

    3. Send to the Troop Medical Forms Coordinator (info below):

    • Physical copy of the forms with physical signature

    • Copy of your current health insurance card.

    • Send documents to:

When there is a campout, a copy of your scout’s health form will be given to the event leader (ASM) for the activity. The event leader should return the physical copy of your scout’s health form to Renee, the medical forms coordinator.

Note: Troop 750 requires this analog system as we do not want our volunteers to keep private information on their personal computers. Again, please keep a copy of your scout’s health forms for your records.
