The Joint Commission's Integrated Care Certification Program
Eligibility: At least one Joint-Commission-accredited care setting, such as a hospital, critical access hospital or psychiatric hospital, ambulatory setting, physician network, nursing care center, or home care organization
About
The Joint Commission offers an Integrated Care Certification program for Joint Commission-accredited care settings. The certification's initial focus is on the design and implementation of a program that is intended to promote and support clinically integrated patient care among the program's hospitals and ambulatory health care providers and organizations. The certification standards emphasize building a strong foundation made up of structures, processes, and a culture that can lead to successful integration and improved patient outcomes. Certification reviews include visits to sites within a program's defined integrated environment. During these visits, reviewers evaluate issues such as clinical integration, hand-off communications, management of information, patient and family engagement, and performance improvement activities that span across integrated sites.Organizations may view the review process guide.
