Identify any and all ways that your efforts have been commended. Do not limit yourself to formal awards in which you received money or a certificate but include promotions and other "pats on the back" where someone else recognized your contributions, i.e., include any recognition you have received for a job well done. Include in your reflection the context in which you received the recognition and the purpose for which it was received. Also, provide details that distinguish your accomplishments or work ethic.
Consider including the following information with each award.
What is the name of the award?
When did you receive it?
Why did you receive it?
Did you compete for the award?
How were honorees selected?
Were you awarded monetary compensation?
Provide any artifacts that support the recognition you received, such as a certificate or photo of key awards.
