Waivers:

Please download and review these.  You will be asked to either complete, sign and bring a copy to the event or sign at the event.

• PCA General Waiver: You will be asked to sign this at the event.

• Minor and Parent Waiver: If you are under 18, please complete this form, have BOTH parents sign it and bring it with you

• Minor Waiver Addendum: Must be completed if you are under 18 and only one parent is able to sign

• Minor and Parent Form Waiver Observers Only: Must be completed if you are under 18 and not driving. One parent can sign.

• Media Release Waiver: ALL attendees and volunteers must complete and sign
