Incident reporting

SafetyNet is a tool to help us meet a key component of an optimal clinical learning environment to continually provide experiences that residents and fellows need to engage with our system’s efforts to address patient safety. Reporting of adverse events, close calls (near misses) is an important mechanism to identify patient safety vulnerabilities. I’ve attached a reference guide listing all of the Incident Types and Incident Categories that can be reported, but some of the most likely to be reported by residents, fellows and faculty include:

 

·         Inadequate communication or communication issue

·         Failure to follow orders, procedures, or protocols

·         Handoff issue

·         Blood or body exposure

·         Communicable disease exposure

·         Difficulty placing IV/vascular access

·         Critical values/significant findings not called

·         Lab specimen lost

·         Adverse drug reaction

·         Incorrect route/form/dosage/administration
