Incoming Freshman, Juniors, those who are new to PH, anyone who did not take in the 23-34 school year
Must have signed parent form or your Privit form filled out
All students will be taking the SWAY concussion testing going into the 2023 school year regardless if previously tested or not. Please read the documents below.
1- Parent Information Letter
2- Concussion ABC's -- Parent Consent Form is on the last page this needs to be printed and brought in on the day of testing
3- "How To" download the SWAY application to your students phone to take the test.
