Objectives
Each student will utilize the Google Docs form application to create a simple survey to gather information about his or her teachers’ favorite books, movies, and TV shows. Each student will then analyze his or her results.

Procedure
Navigate to your Dropbox, create the form inside your Shared Dropbox folder
1 . Open a new form document in Google Docs.
2. Select an appropriate theme and title the form “Book, Movie, and TV Survey”
3. In the form settings uncheck all boxes, it should look like the image below.
4. In the form description, type in the following directions: “Please answer all questions and click Submit. Thanks!”
5. Below the directions box is your first question. Click into the Question Title box and type in your first question, “What is your favorite book?”
6. Next, click into the Help Text box and type “Please type your answer below.”
7. Now use the Question Type drop-down menu and select Text
8. Put a checkmark in the box next to Make this a required question and click Done.
9. Next, click on Add Item (NOT the drop down menu)
10. Click into the Question Title box and type in your next question, “Is your favorite book part of a series?”
11. Now use the Question Type drop-down menu and select Checkboxes
12. Click into the Option 1 box and type, “Yes.” Then click into the Click to add option box and type "No"
13. Put a checkmark in the box next to Make this a required question, then click Done
14. Next, click on the Add Item and select Text from the drop-down menu
15. Now type in your next question in the Question Title box: “What is your favorite movie?"
16. Put a checkmark in the box next to Make this a required question, then click Done
17. Click the Add Item button and select a Grid question.
18. In the Question Title box, type “How many times have you seen your favorite movie?"
19. Next, click into the Column 1 label box and type “1 time.”
20. Use image below to complete your grid question set-up.
21 . Now click into the Row 1 label box and type “Please check one.” Then click Done.
22. Click the Add Item button to insert one more Text question.
23. In the Question Title box, type “What is your favorite TV show?”
24. Put a checkmark in the Make this a required question box, then click Done.
25. Click the Add Item button to insert one more Text question.
26. In the Question Title box, type "What is your name?"
27. Your questionnaire is now complete.
28. To see what your form looks like when viewed by others, click on "View Live Form"
29. Next, click on the Send Form button at the top right corner of your form.
30 . In the Send this form to others window, enter the email addresses of five (5) of your teachers, one being Mrs. Reardon. Then click Send.

* after each email you will put a COMMA and space
32. Your form will now be emailed to your teachers. They will receive an email with a link to the form.
You can click on the See responses button to view the results. Your project is now complete!
