Athletic clearance (PINK Slips):
Please follow the directions listed below to get your pink slip.
Please follow all directions from the Moanalua High School Athletics' websites for current requirements
(Physical form) is the paperwork needed for your YEARLY sports physical. Print out a copy and take it with you to your doctors visit when going for your sports physical.
(Parent consent risk form) is the paperwork needed to complete your YEARLY athletic clearance. Please have your parents complete and sign this form.
Concussion testing for freshmen, Juniors and transfer students. Please check with the Athletic trainers when the next group testing will be.
Then turn the COMPLETED Physical and Parent consent risk forms, ALONG with the $10 Processing Fee (checks Payable to Moanalua High School) in to the athletic trainers office to be processed.
Pick up completed Athletic Clearance (pink slip) from the athletic trainers office ONE DAY AFTER turning in COMPLETED physical and parent consent risk forms and processing fee.
Turn in Pink slip to coach Mark so you can practice and compete!

White and blue Slips:
Will be distributed ONLY at the mandatory parent meeting to be signed and returned as proof of attendance.
Academic Probation:
If you are on academic probation the following must be completed and turned in on the dates specified to be eligible to compete.
If you are on Academic probation, Study hall is MANDATORY!
Please print out and turn in a COMPLETED and SIGNED Grade check EVERY TWO WEEKS TO THE A.D.!
If you do not turn in the grade check on the due date, you are NOT ELIGIBLE to compete until the next grade check.
NO EXCEPTIONS!!!!
