Preseason Forms

All swimmers and divers must complete the following forms and turn them in prior to the first day of tryouts.

• Physical
  • Must have a valid physical on file with school
  • All students must use the official Wake County form
  • Cannot participate in any way without a valid physical
  • Make a copy and give original to Coach Jones

  • Complete the online Google Form

  • Must be completed to ride the team bus

• Student Name/Photo/Video Permission
  • Grants or denies permission for name, photo, or videos to be used by WCPSS
