Membership Application Forms

2. Complete the application, print it on legal-size (standard application) or letter-size (short form application) acid-free paper, and mail it to:

When using the standard application, include documentation supporting descent from the eligible ancestor. When using the short form application, include documentation supporting the claimed lineage from the applicant to the member of the Order who is a qualifying relative on the same eligible ancestor. NOTE: Such shortform applications do not require the $25 a processing fee, but do require either a $20 annual dues payment or a $250 life membership fee.
For standard applications a non-refundable $25 processing fee must also be included. After approval, you will receive a letter of conditional acceptance with four membership options:
A one-time payment of $250 for Lifetime Membership.
A $20 payment each year for Annual Membership.
A one-time payment of $1,000 for Patron Membership.
A one-time payment of $5,000 for Benefactor Membership.
