S & E Registration Forms

Registrations for the Solo & Ensemble Festival must be completed on the downloadable forms at the bottom of this page and mailed in to the site chairman along with payment.  Please click the pop-out arrow on the top left of each form to maximize and download it to your computer for printing.  Also attached is a copy of the Rules & Guidelines for the Solo & Ensemble Festival.

Please note:  All registrations for this festival date only are PAPER REGISTRATION FORMS and must be completed and mailed along with your check (payable to "Michigan District LCMS" with "Lutheran Schools Music Committee" written on the MEMO line) to the site chairperson whose address appears on the Festival Information page (click HERE to return to this page).  Thank you.
