Adult Heparin Protocol
Laboratory 1. Baseline CBC ก่อนหลังได้รับ Heparin
2. Baseline PT กับ INR and aPTT ก่อนเริ่มให้ Heparin
3. ติดตามค่า aPTT ทุก 4-6 ชม. หลังมีการปรับขนาดยา

Heparin Rate Adjustment Sliding Scale:
Ø ตรวจ aPTT หลังจากเริ่มยา 4-6 ชั่วโมง หรือเมื่อมีการเปลี่ยนแปลงขนาดของ heparin ควรปรับขนาดของ heparin ให้ได้ aPTT เป็น1.5-2 เท่าของค่า baseline หรือให้ค่า aPTT อยู่ในช่วงประมาณ 50-70 วินาที ระยะเวลาที่ให้ประมาณ 3-5วัน
Ø Adjust heparin drip as follows: If aPTT < 50, re-bolus patient with specified heparin dose in addition to increasing rate
