Instructions on enabling the form in ms word

1- Click DOWNLOAD THE FORM to start the form download.

2- After the download is complete open the file in MS Word and you will see the following:

3- Click options and the following window will open:

4- Now you will be able to fill the form in word then save (DO NOT SAVE AS) & e-mail it as an attachment.
