Passing along additional information on the new IL Veterans Home in Chicago …………..

The Chicago Home contains 200 single-occupancy rooms

• Private and wheelchair accessible rooms with private bathrooms, including wheelchair accessible showers.

• Skilled care for Illinois veterans, including accommodations for Alzheimer's and Dementia care. Chicago Home does not provide Independent Living assistance.

• Support services, including administration, laundry, dietary services, and maintenance and housekeeping

• 1st floor – resident common areas, administration, facility support departments, outdoor garden area, and large multi-purpose room.

• 2nd floor – Special Needs Unit, including memory care, outdoor terrace with gardens.

• Floor 3-5 – open access, skilled nursing long-term care units.

How do I apply?

• The application process will began on December 1, 2020.

• To request an application, applicants can send an email to dva.chicagovetshome@illinois.gov or contact the facility at 773-628-5905.

What are the minimum requirements for acceptance?

• Honorably discharged veterans who need skilled nursing care.

• Honorably discharged veterans who served in the US armed forces for at least one day of active duty.

• Honorably discharged veterans who have service credited to Illinois or who have resided in Illinois at least one year before the applicant may apply for admission.
