In order to try out for any sport at Holman, each student-athlete must submit the following forms to Mr. Metcalfe or the school's front office. Without these forms, no student will be allowed to try out until these forms are completed. 

1. 2025-2026 VHSL Athletic Physical Form

2. Student Forms (can also be e-signed on PowerSchool)

Complete the VHSL School Physical Form and turn it in to Mr. Metcalfe (Room 2712) before the first day of tryouts. This form must be completed, signed, and dated on or after May 1st, 2025. The insurance company and policy number sections MUST be filled out on the form to try out (Page 2, Part II). 

*Athletic Physicals are different from the school health form. Students will not be allowed to participate without the correct physical form.
