In order to try out for any sport at Holman, each student-athlete must submit the following forms to Mr. Metcalfe or the school's front office. Without these forms, no student will be allowed to try out until these forms are completed.

2. Student Forms (can also be e-signed on PowerSchool)

All student physicals are valid 14 months after the date they are signed by a health care practitioner on page 3. For example, if your child had their physical completed on May 1st, 2025, their physical would be valid through July 1st, 2026. If the date on a student's physical is 14 months or older from the current date, it is expired, and the student is ineligible. 

*Athletic Physicals are different from the school health form. Students will not be allowed to participate without the correct physical form.
