(Home-schooling families, please complete registration form AND contact the district president)

     Green Mountain Music District V invites you to register students for our auditions.

• • The registration form will open after the fall meeting when the process will be finalized. Check the calendar for deadlines

  • Please submit all of your student registrations at once. 

  • If your school has multiple participating teachers, please share the document between your school teachers and submit only one form per school. 

  • If you work at multiple schools, register each school separately. 

  • Students may audition for any performing group and can audition for multiple ensembles, or on separate instruments or voicing ie. jazz tenor sax and wind ensemble alto sax and an alto choral audition as well as a viola string audition.

Directions for filling out the registration form:      

• You do not have editing access to the document

• You must make a copy of the document and name it with your school name and what it is (ie. Falls river middle school reg)

• Fill out all of the information on the teacher page 

• Complete all of the tabs at the bottom with the names of students auditioning 

• Complete the invoice and payment form and click on the link to print out an invoice

• Fill out the invoice and with the check send to our treasurer at the po box listed above or on the invoice itself

• Save your completed file/form in the shared google drive linked on the teachers information sheet
