Candidate Form

Please find below the digital copy of the candidate form (application) for admission into the Glynn Academy chapter of the National Honor Society. You must click the link to open in a new window and then download a copy to be able to edit it.  

Students must complete this form (typed) and return to Dr. Horton no later than Friday February 9th at 4:00 p.m. No handwritten forms will be accepted.

After the deadline of February 9th, a committee of teachers will decide who will become members of our chapter of NHS. Please understand that just filling out the application does not guarantee membership.
