Family ID and Physicals

Students that are interested in participating in athletics in the fall must be registered on the FamilyID system. Part of the registration process is submitting the Section 6: Certification of Authorized Medical Examiner (physical form). Physicals must be dated on or after June 1st, 2022.

A family doctor, urgent care, or minute clinic can conduct physicals. The physical MUST be submitted on the Section 6 form. Please do NOT upload physicals that are NOT on the Section 6 form. The June 1st, 2022 date is a state rule and there are no exceptions.

Some doctors (specifically urgent care) will also want you to see a completed Section 5: Health History form, when conducting the physical. Please DO NOT upload the Section 5 form when submitting the physical. The Section 5 form is available at the following link:

You may upload your Section 6 Physical Form by scanning the form or by taking a picture of it with your tablet or phone. **Do not upload the Section 5 form** Once you have completed your registration and uploaded your child's physical your registration will be reviewed and then approved by our Garnet Valley Athletic Trainers. Your child must be registered and approved prior to trying out or practicing. Please register your child at least 10 days prior to the 1st day of practice so there are no delays in your child being able to participate.
