Registration Form

Fall League 2025.   Registration deadline is September 3rd!   We are starting play Wednesday  September 10, 2025 at 6:00 pm.  Play runs weekly through November 12 , 2025.  Fee is $40.00 per person.    

Please complete the form online below . MAKE CHECKS PAYABLE TO: Forsythe Mixed Doubles Tennis.  You can mail your check to FMDT PO Box 15281, Monroe, LA  71207.  You can also pay via Venmo at: https://venmo.com/u/forsythe-mixed .  

If you need help finding a partner let one of the board members know.  We can’t guarantee  anything, but we’ll try to help.
