Registration Form

Fall League 2026.   Registration is open!   We are getting a small break on the fee from the city.  They are charging us $3.  We are charging $40 for the ten week session and we will provide balls ONLY for Wednesday night play.  We will not be providing balls for any matches you re-schedule for other times.   

Please complete the form online below . MAKE CHECKS PAYABLE TO: Forsythe Mixed Doubles Tennis.  You can mail your check to FMDT PO Box 15281, Monroe, LA  71207.  You can also pay via Venmo at: https://venmo.com/u/forsythe-mixed .  

If you need help finding a partner let one of the board members know.  We can’t guarantee  anything, but we’ll try to help.
