  • Step one :Filling out the Transcript Request Form

    • Fill out your information

    • Place an X next to Official Transcript (give to Mr. Honda/Mrs. Kruse - circle one)

    • Circle

      • Mr. Honda for last names: A-K

      • Mrs. Kruse for last names: L-Z

    • Below Name and address of school/institution/organization... Write information of school you desire to attend

  • Step Two: Take completed form to Business office and pay $1.00. Business Clerk will staple a receipt to completed form

  • Step Three: Take the request form/receipt to the Registrar's office. Once the form is processed the registrar’s office will place the form in Mrs. Kruse/Mr. Honda box.
