California Designated Representative Training Programs - Find Approved Courses

One of the required forms that must be included in your license application package is the proof of required training, specifically, the Training Affidavit form.

The California Board of Pharmacy will accept a Training Affidavit that is completed and provided by a training program approved by the California State Board of Pharmacy (The Board).
