Documents to be provided for the establishment of ordinary passport

I. Burkinabè Ordinary Passport Application Files

1. complete the passport application form,

2. one (1) extract of birth certificate or a legalized copy thereof,

3. one (1) certificate of Burkinabe nationality or a legalized copy thereof,

4. one (1) legalized copy of the Burkinabè identity card or the consular card,

5. one (1) criminal record dating back less than three (3) months,

6.    three (3) identity photos taken from the front on a white background, size 4.5 cm by 3.5 cm. Wearing earrings and corrective glasses is allowed.

7. one (1) certificate of profession or a legalized copy thereof, or a legalized copy of the student card;

II. Application files for ordinary Burkinabè passport

FOR PERSONS HAVING ACQUIRED NATIONALITY THROUGH MARRIAGE

1. complete the passport application form,

2. one (1) extract of birth certificate or a legalized copy thereof,

3. one (1) certificate of Burkinabe nationality or a legalized copy thereof,

4. one (1) legalized copy of the consular identity card,

5. one (1) criminal record dating back less than three (3) months,

6. one (1) legalized copy of the spouse's passport, CNIB or consular card,

7. one (1) legalized copy of the marriage certificate,

8.    three (3) identity photos taken from the front on a white background, size 4.5 cm by 3.5 cm. The wearing of earrings and corrective glasses is allowed,

9. one (1) certificate of the profession or a legalized copy thereof,

10. or a certified copy of the student card,

III. Application files for an ordinary Burkinabè passport

FOR A MINOR CHILD

1. complete the passport application form,

2. one (1) extract of birth certificate or a legalized copy thereof,

3. one (1) certificate of Burkinabe nationality or a legalized copy thereof,

4. one (1) parental authorization (father or mother only),

5. one (1) certified copy of the father's and mother's passport, Burkinabè identity card or consular card,

6. one (1) residence permit abroad for the parents,

7. one (1) school certificate for pupils and students,

8.    three (3) identity photos taken from the front on a white background, size 4.5 cm by 3.5 cm. The wearing of earrings and corrective glasses is allowed,

IV. passport renewal application files ordinary Burkinabè

1. complete the passport application form,

2. one (1) photocopy of passport and/or declaration of loss if applicable,

3.    three (3) identity photos taken from the front on a white background, size 4.5 cm by 3.5 cm. The wearing of earrings and corrective glasses is allowed,

4. one (1) birth certificate or a certified copy thereof (optional),

5. one (1) certificate of Burkinabe nationality or a legalized copy of it (optional),

NB . :

the passport can only be renewed if its validity does not exceed three months ,

in the event of loss of passport, attach the declaration of loss, for passports issued before 1991, create a new file, for any request for change to be made, attach supporting documents.

NB :

-        The documents must be in French and, if necessary, attach the French translation approved by the Embassy.

-        Fees for issuing or renewing the ordinary Burkinabè passport 12,000 Yen (50,000 FCFA); to be paid to the treasury of the Embassy of Burkina Faso in Tokyo.
