Derby City Council has undertaken a review of its pay and grading structure and terms and conditions.
This post is in scope of the review and has been evaluated at Grade G £24,472 - £27,123 a year to be effective from the implementation of the new pay line and harmonised terms and conditions of employment from 1 June 2016. The successful candidate will move to that grade on implementation. Anyone starting before the implementation will be appointed on the old grade which is Grade 8 £27,924 - £30,978 a year
The Livewell programme is a nationally recognised holistic healthy lifestyle programme, which addresses health inequalities through an offer of community NHS Health Checks, motivational interviewing, physical activity, nutrition, stop smoking services and health education.
We are looking to appoint a passionate and enthusiastic Coordinator that will lead and manage the delivery of community NHS Health Checks within Derby City. The post holder will also have a crucial role in the wider Livewell service management team that will ensure the service outcomes are met.
The Livewell co-ordinator will also lead and manage a team of Livewell advisors who work with a caseload of clients wishing to change their behaviour to result in weight loss, increased activity levels and live smoke free if applicable. The role is required to engage and support, health professionals to refer patients to the Livewell programme and develop and coordinate a Livewell programme that engages with local communities, to participate within Livewell community hubs.
You will be creative, passionate about working with people and communities to support long term behaviour change, have a keen eye for detail able to work as part of a team. Post qualification experience of leading and developing teams and people with the ability to multi task across the service.
A high level of understanding and experience of delivering and managing community NHS Health Checks is essential for this role.
