Purpose: The purpose of this announcement is to provide staff with information on a housing opportunity for individuals with disabilities.

Description: The Section 811 program allows persons with disabilities to live as independently as possible in the community by subsidizing rental housing opportunities which provide access to appropriate supportive services.

Eligibility Requirements:

- Adults ages 18 and older

- Disabled as defined by HUD

- Low-Income

- Eligible for Long Term Services and Supports through Medicaid

Priority Populations:

- Institutionalized: nursing facility, prison, state psych hospital or hospital where assistance is available under Medicaid

- At risk of institutionalization due changes in health status, available supports, poor housing conditions

- Moving to independent renting: DDA Community, Brain Injury waiver ALU, Residential Rehab Program

- Homeless in this order: Actually homeless, Imminent risk of homelessness, Fleeing domestic violence
