Documents required for AoS:

1. Driver License

2. ATM Card

3. Medicare Card

4. 2 x payslips/employment letter

5. ATO NOA(2011)

6. Letter from DIAC

7. Passport with Visa or Citizenship certificate

8. Signed form: sign page 9; sign page 10 in front of a Centrelink officer

Note:

1. Provide documents to any Centrelink branch and sign form page 10 in the front of an officer (within 14 days after phone interview)

2. A letter requesting for Bond will be posted by Department of Human Services after interview

3. Pay bond in a CBA bank and submit Bankguarantee paper & AccountConfirmation details to DHS？？

4. AoS will be finalized within 42 days

5. If extension needed, assurer will need to contact DIAC request for the extension.

6. No appointment required to submit those documents to Centrelink branch. Bring original documents and Centrelink will make copies then return originals.
