Note that-
For fresh/re-issue/renew passport, FORM 1 is required to be filled.
For changes/correction in name or other information/addition or deletion of spouse name , FORM 2 is required to be filled.
In case of change of address, fresh booklet is issued by the concerned RPO. So, in that case applicant is required to fill up FORM 1 with complete and updated informations.
If the applicant has been living for less than 1 year at the address mentioned in the application form, he/she needs to fill up 2 additional set of PERSONAL PARTICULAR FORM (PPF), providing previous address details along with proof of stay.
It should be noted that in some cases, ANNEXURES are required to be given along with passport application form. The same can be downloaded from the below attachment section.
Here are the list of ANNEXURES required to be provided :
ANNEXURE 'B' - ALL CENTRAL GOVERNMENT EMPLOYEES STATE GOVERNMENT EMPLOYEES, EMPLOYEES OF STATUTORY BODIES AND PUBLIC SECTOR UNDERTAKINGS AND THEIR DEPENDENT FAMILY MEMBERS ARE REQUIRED TO PRODUCE A IDENTITY CERTIFICATE.
ANNEXURE 'F' - SPECIMEN VERIFICATION CERTIFICATE (FOR PASSPORT UNDER TATKAL ONLY)
ANNEXURE 'H' - DECLARATION OF APPLICANT PARENT OR GUARDIAN IF PASSPORT IS FOR MINOR: ON PLAIN PAPER.
ANNEXURE 'I' - TO BE EXECUTED ON APPROPRIATE NON-JUDICIAL STAMP PAPER AND ATTESTED BY A NOTARY PUBLIC, ONE ORIGINAL AND ONE SELF-ATTESTED PHOTOCOPY
AUTHORITY LETTER TO REPRESENTATIVE - Its needed to be given to the authorised representative who submits application for passport on behalf of the applicant.
