Jackie is our care coordination and scheduling persona. It's one email that is monitored by our team no matter the day! 'Jackie' can handle any question or request from scheduling to billing to clinical. You can also log into your Family Room and request a visit change/hour change by using the "To Do" portion. This is for non-urgent requests only; we don't want a delay in reacting if your visit change is urgent! Cat is a wealth of knowledge with Care Coordination and Scheduling. She's our primary "Jackie" during office hours.
The Silver Lining Home Healthcare Family Room portal is one place where you can see your/your loved ones schedule, send tasks/requests to the Silver Lining team, view your invoices, and see what has or has not been done at a visit.
All services come with our Reliability Pledge - if we're late, we pay you!
We currently offer:
Companion Care - Assistance with errands, transportation, laundry, light housekeeping, cooking
Personal Care - Assistance with dressing, bathing, hygiene, transfers and medication reminders
Nursing Care - Medication Management and Administration, PEG Tube care, catheter care, education, infusions
All care is overseen by our Clinical Team, and Clinical Director Amy Clark is a Certified Dementia Practicioner.
